PLEASE TAKE NOTICE that a member has indicated a desire to obtain a copy of the Association’s membership list (including each member’s name, property address, mailing address, and email address). California law requires the Association to disclose this information to a member following receipt of a written request that complies with the law, BUT the law also gives members the right to “opt-out” of having their name and addresses (including email address) information included in the membership list that other members may obtain. The choice to opt-out must be in writing. Therefore, if you do NOT wish to have your private information of name, property address, mailing address, and email address information included in the Association’s membership list that other members may obtain, you MUST request the Association in writing to withhold such private personal information from the membership list.  To Opt-Out of sharing your name, property address, mailing address, and email address with other members, please complete this form to the Association’s managing agent as soon as possible.

This Opt-Out authorization shall remain in effect until changed via written notice to the Association’s managing agent. THIS OPT-OUT DOES NOT AFFECT DISTRIBUTION OF ANY NOTICES SENT BY THE ASSOCIATION.
